Appointment times are Tuesday through Wednesday (1:00pm to 8:00pm), and Thursday (1:00pm to 7:00pm) to schedule an appointment, please call 317-840-0490 (Tuesday through Friday 9:00 am to 6:00 pm). Phone Counseling & Online Counseling Hours
For added convenience, these appointment times are also available during regular office hours plus Tuesday through Friday 10:00am to 6:00pm. Virtual counseling removes barriers of long distances to travel, traffic jams and can be done in the comfort of your own home. I’m licensed in both California and Indiana for mental health services.
All sessions are 50 minutes and $165 each. I accept cash, check or credit cards.
At this time I accept Anthem Blue Shield/Blue Cross (both PPO and Federal Program) until 12/22/17. For all other insurances, I am considered an out-of-network provider. When requested I will generate an invoice for you to submit to your insurance company for possible reimbursement. Please contact your insurance plan to confirm I am an in-network provider and to confirm your “mental health benefits at an office setting.” If you are wanting to do online counseling then ask about your “telehealth” benefits.
After our initial phone consult, to save you some time when you come in for your first appointment, please click on the Secure Client Area to complete the required documentation. If you are a parent or legal guardian, you will need to complete the Consent to Treat a Minor form. If you are a parent who is separated or divorced I will need signatures from both parents before the child can begin treatment.
